In case of fresh or reissue passport or in case of
loss of passport, the application form may be
submitted at the Passport Office (after on-line
registration) or the applicant may do a walk-in
submission at e-Seva centers, Speed-Post Offices,
Collection Centres in the District Superintendent of
Police offices, Passport Application Collection
Centres (PACC) Tirupathi / PACC Vijayawada. Tatkal
applications must be submitted at the Passport
Office only after online registration.
For Miscellaneous services like inclusion / deletion
of spouse name, correction in personal particulars,
change of address, renewal of short validity
passport (a passport issued with short validity
viz., 6 months, 1 year, 5 years etc), PCC, ECNR etc,
the application has to be submitted at the Passport
Office only. Online registration is not required.
PCC and ECNR services need
form no.2, rest should be submitted in
form no.1.
For all other categories of applications (ordinary
or Tatkal, mentioned in above para) online
registration is a must if you wish to submit at the
passport office. |

I need
passport in a week’s time. How do I get it ? |
|
You may apply in tatkal service on the basis of
either a Special
Verification Certificate or three out of
Fifteen Documents
prescribed by the Government along with standard
affidavit Annexure I. Subject to all documents being
in order, passport will be dispatched by speed post
in a week’s time. For Tatkal service, you need to
register online at
www.passport.gov.in and then submit the
application at the passport office in Secunderabad.
Please note that presently Tatkal applications are
not accepted anywhere else. You need to submit the
application in person with all supporting documents,
originals should also be shown at the counter. |

Does the applicant need to come in person to apply
for Ordinary passport, Tatkal, Miscellaneous
passport? |
|
In all the cases, the applicant is required to come
in person to submit the application form (except for
minor applicants where at east one of the parents
should be present). However, in exceptional cases
(on health grounds etc) an authorized representative
with identity proof my submit passport for ordinary
or miscellaneous services. The applicant also has
the option to submit through Travel Agents
authorized by the Passport Office.
However, this does not apply for Tatkal
cases. |

What do I do if I have a Court case/ criminal case
against me? |
|
As
per the Passports Act, no passport is issued to a
person if it comes to the notice of Passport
Authority that a criminal case is pending in a court
of law against that person. Passport is also not
issued if a person is convicted in the last five
years with imprisonment upto two years and above.
However, Gazette Notification 570(E) dated 25th
August 1993 permits issue of a short validity
passport to a person against whom a criminal case is
pending if he produces the permission of the
concerned court for departure from India.
Give full details in the appropriate columns in the
application from. Further, obtain a Court order
granting permission for travel abroad, which should
be duly attested in original by the Court and
enclose the same along with the passport
application. If there is more than one case, the
order should cover all the cases. Submit an
undertaking to follow the Court's directions for
travel abroad as per the terms laid down and to
surrender the passport as directed any time by the
authorities concerned.
Please note that suppression of any material
information relevant to the issuance of passport is
an offence under the Passport Act, 1967. |

I am a student staying away from my parents. Where
do I apply for passport and what proof of residence
shall I have to furnish ? |
|
For example, if you are a student staying in
Warangal and your parents are staying in Karimnagar,
furnish along with your form, four Personal
Particulars forms. Please also indicate your hostel
address/the place where you are staying in Warangal
as present address and your parent’s address as
Permanent address. In case you are studying and
staying in Bangalore, you can either apply at the
Passport Office Bangalore or in the Passport Office
in whose jurisdiction your parents’ address
(permanent address) is located, e.g., Hyderabad. In
the later case, fill up an additional set of

form giving the address in
Bangalore. For proof of residence at your
hostel/place of stay, you should provide either a
Bonafide Certificate issued by the Principal/Warden,
or, proof of residence of the guardian with whom you
are staying along with the guardianship affidavit.
In case your original certificates (SSC etc) are in
the custody of the educational institution, you
would also need to furnish a custodian certificate
from the authorities along with photocopies of the
certificates. |

Till six months ago, I was working in Guntur. I have
now shifted to Hyderabad. What documents do I
enclose while applying for a fresh passport? |

Mentioning your Hyderabad address in the present
address column no. 9, you should also mention your
Guntur address in column no. 9. a. where you are
asked to furnish your address if you have been
residing since less then one year. You should also
furnish two additional personal particulars forms
indicating your previous address. This should be
furnished along with other requisite documents. |

I had applied for fresh passport three months back
in Mumbai while I was working there but, did not
receive my passport. I am now urgently in need of
my passport and my file is pending. Can I apply in
Hyderabad, if so, how? |

You may apply in Hyderabad by furnishing details of
your Mumbai application in the relevant column of
the form. Suppression of material information
regarding your previous stay will lead to imposition
of penalty of Rs. 5000/. |

My passport was issued at Hyderabad 10 years back. I
have been working in Delhi for last 5 years and
moved back to Hyderabad 2 months back. I now want
to renew my passport. Where should I apply, whether
in Hyderabad or Delhi ? |

You should apply in the Passport office in the
jurisdiction of your present address only. Addresses
should be clearly mentioned in column nos. 9 and 9a.
Proof of stay on present address should be
furnished. In case no proof is available as per list
prescribed by the Government of India, you may
obtain a Residence Certificate issued by the Mandal
Revenue Officer. Please note that period of
stay and addresses should be mentioned as
specifically as possible. |

You may report about the loss of passport at the
Police Station concerned and along with the loss
certificate issued by them, you may apply for a
fresh passport on form no. 1, enclosing copy/details
of your lost passport. If there is any change in
your present address / any other particulars,
requisite documents should be furnished as
prescribed by the Government of India. |

My passport got
damaged during the heavy rains few months back. How
should I get a new one ? |

You may apply for a new passport by submitting form
no. 1. You should furnish your damaged passport at
the time of submission. If there is any change in

I don’t have ECNR stamp in my Passport. Can I get it
on the basis of my provisional matriculation or
above certificate issued by the University/Board
concerned? |

We are no longer affixing ECNR stamp in passports.
In fact it is not even printed in the new passport
booklets. Currently, only ECR stamp is being put in
passports if applicant is not eligible for ECNR.
However, you are entitled to ECNR status if you
possess qualification of Matriculation or above.
Even you can get ECNR status on the basis of
provisional matriculation certificate if the mark
sheets of all semesters of your course are available
with you. |

Address change :: |
|
I got my passport from Delhi. Now I have shifted to
Hyderabad. Should I change my address in the
passport? If change is required, what are the
documents to be furnished and how much time will it
take? |
|
It is up to you to change the address. It is always
better if your passport indicates your current
address. For this purpose, you need to fill up
form-1 and submit it along with valid proof of your
residential address at Passport Office of the area
of where you reside. |

Change of spouse name :: |
|
I am holding a passport which includes my husband’s
name. Now that I am divorced, how do I get my
husband’s name deleted from my passport ? |
|
You may apply for a new passport under miscellaneous
category requesting for deletion of spouse name.
Divorce certificate issued by a competent authority
should be submitted along with the application form.
In case of change of address or any other
particulars, the requisite documents may be
furnished. In case you would like to change your
surname subsequently you need to submit (i) Divorce
deed duly authenticated by Court and (ii) Deed Poll
or Sworn Affidavit as per specimen in Annexure –D on
non-judicial stamp paper attested by a Notary. |

Corrections in passport:: |
|
If there is any mistake in the personal particulars
of the applicant in his/her passport, the same may
be submitted at the Passport Office for correction
at the Correction Counter. If the mistake was made
by the applicant in the original application form,
then he/she would have to apply for the correction
in category ‘R’ by submitting form no. 1 and a fee
of Rs.1000/ would be charged. Online registration is
not required in this case. |

the passport of my minor child. I want to include my
present spouse name in his passport. What are the
documents required? |
|
In event of remarriage after divorce, the biological
parent’s names will only be entered in the
passport. However, if the stepfather/stepmother is
appointed by the court as a legal guardian, the name
of such stepparent can be written as legal guardian.
In the event of remarriage after death of the spouse
the name of stepfather/stepmother may be written in
the passport against the column of father/mother. |

I want to change my date/place of birth in my
passport. What documents are to be enclosed? |
|
When a competent authority which issued a birth
certificate (Municipal authorities) or SSC
certificate, issues any correction or amendment in
date/place of birth, no court order is required (if
the same document was submitted earlier for the
passport), However, fresh fee will be charged and an
affidavit to that effect should be given. In case
there is a conflict between the birth certificate
and the SSC, birth certificate usually prevails.
Supporting documents submitted at the time of your old
application would be compared with the documents
submitted with the new application. A declaratory
order from a First Class Judicial Magistrate of a
Civil Court may be required. Please note that
suppression of information/ providing of wrong
information is an offence under clause 10(3) (b) of
the Passport Act. |

In case of a valid passport for more than one year,
a sworn affidavit has to be given for change of
appearance.
In case of requests from Sikhs who want to change
from turbaned photograph to clean shaven ones, or
any other major visible change in others, a
notarized statement or certificate from any of the
authorities authorized to sign verification
certificates should be submitted. Fee will be
charged for a fresh passport. |
